C3 Intake form for patients and caregivers

The Canadian Cholangiocarcinoma Collaborative (C3) is a not-for-profit partnership among patients, caregivers, oncologists, and researchers which was formed to grow hope in patients and families affected by biliary tract cancer (BTC), including cholangiocarcinoma and gall bladder cancer. The C3 was founded to change how patients with BTC receive and access integrated research and care in Canada. We aim to advance research of BTC, increase the number of clinical trials available in Canada, enhance access to molecular testing, advocate for access to therapies that may benefit BTC patients, grow a nation-wide network of physicians treating BTC, and provide information and support to those affected by BTC in Canada.
